You may request an absentee ballot by mail for yourself or a near relative (spouse, brother, sister, parent, grandparent, child, grandchild, mother-in-law, father-in-law, stepparent or stepchild).
TO REQUEST AN ABSENTEE BALLOT BE MAILED TO YOURSELF OR A NEAR RELATIVE:
Mail a letter to the Board of Elections Office stating that you wish to receive an absentee ballot by mail and include the following information:
- Your name
- Your address
- Your date of birth
- The address where the absentee ballot is to be mailed
- Your signature -the letter must be signed by the voter requesting the absentee ballot or by the near relative requesting the absentee ballot
- If you are a near relative requesting the absentee ballot please list relationship to the voter. Those acceptable are: spouse, brother, sister, parent, grandparent, child, grandchild, mother-in-law, father-in-law, stepparent or stepchild.
***Please note: A POWER OF ATTORNEY is not allowed to request an absentee ballot for another voter unless the voter is a near relative as described above.
