APPLICATIONS FOR SERVICES

If you are wanting to complete an application for services with our Department, you must do one of the following:

• Click on the appropriate application and fill out all information on-line, and return application to our office (some applications MAY be able to be sent electronically); OR
• Click on the appropriate application, print it out, complete all information, and return the application to our office; OR
• Visit the Environmental Health office located at 209 N. Graham-Hopedale Road, Burlington, NC  27217, and obtain and complete the appropriate application.

Please keep in mind that the application must be returned to our Department with the corresponding fee.

SEPTIC SYSTEMS AND WELLS
Please click on the application you need below:

• Soil/site evaluation (needed if you are wanting a new septic system/Improvement Permit/Authorization to Construct)
• Septic system repair (needed if your septic system is currently malfunctioning)
• Well permit (needed if you are wanting to drill a new well)
• Well repair permit (needed if you are wanting to repair your well)
• Water samples (needed if you are requesting water to be sampled from your well)
• Well abandonment permit (needed if you are wanting to close/seal off an existing well)
• Existing septic system inspection (needed if you are replacing a mobile home or are needing power restored to a home or business)
• Well camera (needed if you are wanting our Department to camera your well, or if you want to use thermoplastic casing in your well)

RESTAURANTS (Food & Lodging)
