Eligibility Information

 

Eligible applicants must:

 

� Be a U.S. Citizen or qualified alien

� North Carolina resident

� Have a social security number or have applied for one

� Have income that meets program requirements for one of the following categories:

�  Families with children under age 19

�  Children under age 21

�  Single or married pregnant women

�  Children receiving adoption or foster care assistance

                                    OR

� Be currently receiving a check for Supplemental Security Income (SSI), Work First Family Assistance (WFFA), or Special Assistance for the Aged, Disabled or Blind (SAA, SAD or SAB)

� Be uninsured 

� Be members of families at or below 200% of the Federal Poverty Limits.  The amount of money families can earn and still qualify depends upon the number of people in each family unit.

� Be ineligible for Medicaid, Medicare or other federally sponsored health insurance.

� Pay the enrollment fee if applicable. (Families whose income falls between 151% and 200% of the Federal Poverty Limits must pay an enrollment fee of $50 per child up to a maximum of $100 per family.  The enrollment fee must be paid each year.)
