At Risk Case Management

Our At Risk Case Management services provide assistance to Medicaid-eligible abuse or neglect, homelessness, or placement in an adult care home or nursing home.

The purpose of case management services for adults at-risk for abuse, neglect or exploitation is to assist them in gaining access to needed medical, sociak, educational, and other serivces, to encourage the use of affordable medical care by referrals to appropriate providers; and  to discourage over using costly services.  To be eligible for this service, an adult must need help with bathing, dressing, grooming, toileting, transferring, ambulating, eating, communicating, and not be receiving assistance with these activities from anyone else on a regular basis.  If you know somone who needs help with managing financial affairs, shopping, housekeeping, laundry, meal preparation, using transportation, using a telephone, reading, writing, and they do not have anyone else to help them, they also may be eligible for this service.  You also must be receiving Medicaid  or apply and be approved for Medicaid to receive this service.
